A 15-year-old healthy black female presented with persistent white spots that had been present since the patient was 4 years of age. Her father had a similar condition. The physical examination revealed scattered circinate hypopigmented to pink macules across her forehead (), neck, and back (). She also had unrelated hyperpigmented scars from a previous chicken pox infection. Shave biopsy specimens revealed coarse hypergranulosis and enlarged keratinocytes with pale blue cytoplasm ().
